Job Summary
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The LPN will provide high-quality nursing care to patients, particularly in memory care and geriatrics settings. This role involves collaborating with registered nurses and other healthcare professionals to ensure the best patient outcomes while adhering to established protocols and standards.
Duties
Administer medications and treatments as prescribed by physicians.
Monitor patient vital signs and report any changes to the healthcare team.
Assist with daily living activities, including tube feeding and personal hygiene.
Maintain accurate patient records using electronic health systems such as Cerner.
Implement infection control measures to ensure a safe environment for patients.
Provide support and education to patients and their families regarding health management.
Participate in care planning meetings and contribute to the development of individualized care plans.
Assist in diagnostic procedures, including sonography when necessary.
Lift and transport patients safely while ensuring their comfort and dignity.
Qualifications
Valid Licensed Practical Nurse (LPN) license in the state of practice.
Experience in a clinical setting, particularly with geriatrics or memory care preferred.
Knowledge of medical terminology and infection control practices.
Proficiency in using electronic health record systems, specifically Cerner.
Strong communication skills, both verbal and written, with the ability to work effectively within a team.
Ability to lift patients safely and assist with mobility as needed.
