Job Description:
Responsible for planning, coordinating, providing care and enrichment services for elderly members of Senior Program to assist them in improving their lives
- Develop and coordinate enrichment activities and outings for members under Center Director’s supervision
- Oversee overall program activities to ensure the compliance with the Center policies, goals
- Perform initial and ongoing assessments on members to evaluate current support network including family members, living arrangements, financial status, physical health, level of self-care
- Collaborate with social workers, counselors, health service providers
- Develop personal connections with members to promote and encourage healthy living
- Provide support to members in their emotional and physical difficulties
- Assist members in identifying needs to connect them with appropriate resources and referrals
- Assist in completing program-required paperwork per Senior Program documentation policy
