Objective:
Seeking a CNA, Med-tech or phlebotomist position in which my skills and strengths in medical care will be of value.


Experience:
December 2004– Present: Bayada Home Health, CNA

Responsibilities: Giving assistants with a Client/CNA ratio of 1/1 in the client’s home. Giving assistance of ADL, cooking, housework, charting clients condition on a daily basis. Having a close relationship with staff supervisors in discussion of clients change in state or condition.

May 2004-January 2005: Walnut Ridge assistant living facility, CNA/Med-tech/supervisor

Responsibilities: Giving assistants to residents in an ADL atmosphere, such as grooming, bathing, dressing, feeding, transferring, and ambulating. Administration of medications and at the same time overseeing other employees as a supervisor. Reviewing FL2 forms, Intake and output, medication administration records, diagnosis, admitting and discharging residents. Previewing physician orders and transcribing to MARs.

Responsibilities: Giving assistance to residents in an ADL facility, also with the responsibility of administering medications.
June 1999–April 2003: Creekside Manor assistant living facility, Personal care assistant/med-tech as of 2002

Responsibilities: ADL assistance and administering medications.
April 2003-July2003: Christian care of Winston-Salem, CNA/Med-tech/relief supervisor

Responsibilities: All ADL skills provided and administering medications.

I am a very dedicated health care professional with more than four years experience helping elderly patients with activities of daily living. I have worked extensively with FL2 documents, physician orders, admitting and discharging, pharmaceutical changes, medical terminology, and medication administering, also very familiar with resident/patient charts, etc. I am a very focused, goal oriented, as well as being accountable and well organized.
