I have approximately 4 years experience as a case manager as well as 2 1/2 years experience as a quality improvement coordinator Case Manager / Patient Care Coordinator
Duties included discharge planning, utilization management and review, auditing Medicare charts for appropriate admission, continued stay and discharge criteria, auditing patient charts for core measures, participating on multidisciplinary committees, daily communicating with physicians, patients and their families, multidisciplinary health team and outside facilities to correlate appropriate inpatient care and appropriate discharge related to patient needs. Quality Improvement Coordinator
Duties included heading quality improvement programs related to resident care and facility wide improvement, risk management and daily investigation of resident incident reports, infection control and auditing resident medical records.
