Patient Relations Representative/Team Leader: v Team received in coming calls from patients/sparrow associates & put into service response database. Contact & coordinate patient, housekeeping, repairs & maintenance needs as quickly as possible. Provide a “One Stop Shopping environment”. Follow up as needed & most of all make the patients stay as comfortable as possible. v Contact daily (45-70) patients after discharge (approximately 2 days) to interview for satisfaction. Contact is to determine if they received discharge instructions, current condition, any issues/problems good or bad during stay and would they recommend Sparrow. Document information in Excel spreadsheet. Forward report results to treatment floor, supervisors, upper management, Patients 1st Council and OASIS team. (95%) completion rate. v Receive approximately (60) calls daily from nursing associates on all units requesting transport for patients for discharge, deliver patients to other units, 02 tanks, deliver blood, wheel chairs, medical charts, specimens to lab, etc. Information is entered into transport tracking database. Team monitored transport database for conflicts, errors and number of jobs to be completed, appropriate calls made to resolve issues.
