-Maintain a case load of 84 residents in a 250 bed facility (ie; initial assessment, 14 day reviews, quarterlies, significant changes, annual reviews, family meetings, care plan meetings, and mds)
-Organize and facilitate individual group programs with voluntary residents
-Responsible for arranging and planning weekly community outings (ie; shopping trips, baseball trips, football trips, special events)
-Operate and drive a 15 passenger wheelchair bus for the special events (listed above)
-Work with an interdiscipinarity team of MDS Coordinator, Nurses, Occupational Therapists, and Social Workers to determine appropriate level of care for each resident
-Implement and deliver recreational activities to enhance the resident's quality of life
