Job Description:
Obtains medical history, particularly as it relates to the present condition, from the patient and/or family member.
Provides skilled nursing care as outlined in the nursing care plan to include, but not be limited to the following:
Coordinates and supervises the care given by the LPN, Home Health Aide and/or CNA assigned to provide care to the patient.
Charts those services that has been rendered to the patient, makes revisions in the nursing care plan as needed, records supervisory visits, evaluates patient care and progress and closes charts of discharged patients by preparing Discharge Summaries and obtaining Discharge orders.
Maintains confidentiality of client’s situation, records and services provided according to HIPPA regulations
Other office duties and supervision of direct patient care
Job Requirements:
Registered Nurse candidates will have strong critical thinking skills, one year of home health care experience, registered nurse in the State of Illinois, must work well independently, enjoy problem solving and understand the importance of accurate documentation.
Knowledge of all applicable federal, state and local laws and regulatory requirements
Illinois Registered Nurse License is required.
Computer skills are required.
