Job Description: The Case Manager provides services and support to persons who are deaf-blind at the Helen Keller National Center. The case manager ensures the services provided to students are person centered and meet student needs through assessment, observations, discussions and direct contact. The case manager collects information regarding the studentˇ¦s program and communications to all team members student training priorities, short and long term goals and ensure studentˇ¦s involvement in the decision making process. Other duties include:
„« Serves as the primary liaison with sponsoring state agencies and other outside services „« Chairs regularly scheduled evaluation, training and case conferences „« Compiles all department reports and prepares written documentation to sponsoring agencies including recommendations for rehabilitation training „« Provides personal communication and vocational counseling services to students individually or in groups „« Maintains appropriate documentation (i.e. case notes, authorizations, record of student accounts, etc) „« Coordinates personal leaves, transportation arrangements and other support services to students
Job Requirements: „« Minimum of two years experience in related field required „« Masters Degree required in Rehabilitation Counseling or Related Field. CRC preferred. „« Knowledge of deaf-blind community, vocational rehabilitation and job placement a plus. „« Proficiency in American Sign Language and other methods of communication utilized by individuals who are deaf blind preferred „« Must be organized, detail orientated, and a team player „« Some travel required (local and out of state when necessary)
