Job Summary: The position provides coordination of continuing care and case management services for patients including developing a continuing care plan, supervising and teaching relapse prevention, life skills, re-entry skills, family education on relapse symptoms and coordinates service with other care providers. The primary function of this position is to provide transitional and outreach services to patients who have completed the Old Minto Family Recovery Camp program, with the objective of assuring provision of continuing care services individuals and their families after their return to their home community following completion of treatment services. Qualifications: EDUCATION: High school diploma or GED equivalent; AND level 1 Alaska Chemical Dependency Counselors Certification or equivalent combination of education and work experience. Certification required within 12 months of hire.Must have a current Alaska State Drivers License.Familiarity with Alaska Native Traditions and cultures.Able to travel for extended period with short notices.FBI background check.One year sobriety required, U.A. monitored.
