RESPONSIBILITIES: ? Provides key input to the initial assessment of consumer need and development of treatment/service plan. ? Provides key input of treatment/service plan for selected consumers by providing a variety of clinical, educational, and recreational services. ? Completes consumer progress logs. ? Performs routine administrative duties associated with specific clinical settings (medication management, prepare menus, health care coordination, school meetings, etc). ? Establishes and maintains collaborative relationships with appropriate community resources (i.e. schools, family/community support). ? May perform the following duties: ? Residential treatment as part of a therapeutic team requiring 24 hour supervision of consumers. ? Day treatment as part of a therapeutic team requiring time limited supervision of consumers. ? Provides backup to supervisor with other related activities/duties as required.
