Conduct assessments and interventions on Humana participants in their place of residence (home, AL, LTC) Provide information, education and resources on health conditions, preventative services, and community resources Coordinate care and services as deemed appropriate Act as a field liaison between Humana, Green Ribbon Health and the community Work collaboratively with the interdisciplinary teams from Humana and Green Ribbon Health Understand clinical program design, implementation, management, monitoring and reporting NOTES: 10 openings.
