Job Description:
The Home Care RN coordinates the patients' and families’ care and services including those of the team Social Worker, Chaplain, and Physician. He/she provides skilled nursing as determined by the interdisciplinary team plan of care, participates in weekly team meetings, teaches families and other primary care persons appropriate care techniques and provides accurate documentation with visit itineraries.
Job Requirements:
Qualifications include being currently licensed as a Registered Nurse in PA w/ a minimum of 2 years nursing experience in either med/surg or oncology. Must have reliable transportation.
