Job Description:
Leads a care management work team of nurses, social worker, and nurse practitioners that creates high levels of continuity of care, member satisfaction, and partnership with Permanente and community physicians. Addresses and resolves systems issues through appropriate policy channels within the organization. Directs a team who uses automated systems for documenting and monitoring patient care activities. Implements a systematic approach for ensuring effective use of resources, maintaining standards of care, and achieving desired patient outcomes. Develops and maintains clinical operational relationships with internal and external renal care providers who care for members with ESRD. Facilitates successful relationships across institutional boundaries, with sensitivity to the clinical care and operational needs of each provider. Develops and implements care management policies and procedures within the department and collaborates in the process in other settings when indicated. Participates in strategic planning and priority setting for the department. Assures that care management programs are operating at or above the standards required by NCQ A , MDQR, and other regulatory organizations. Collaborates with key stakeholders and support services to ensure coordinated service delivery system support for medical center operations in the implementation of meaningful systems and programs. Is responsible for implementing a systematic approach for communicating with physicians, clinical teams, and regional administration about care management activities. Develops, executes, and evaluates work plans for Care Management. Directs a team whose work plans are aligned and contribute to the integration of evidence-based practice into the daily work of health care delivery teams. Consults on methods for electronic dat a interface through the automated medical record.
