Job Description:
The role of the Care Manager is to conduct activities that focus on the coordination and integration of services needed by sick newborns and their families during hospitalization and thereafter. The Care Manager is to assure quality of care, optimal clinical outcomes and facilitation of cost-effective resources utilization. The Care Manager utilizes all available resources to achieve optimal outcomes. Areas of responsibility include: assessment of care management needs, developing care management plans and expected outcomes, coordination of care, monitoring and evaluation of outcomes, and communication. Qualified individuals should be appropriately licensed RN with 5 years of clinical NICU experience required, utilization review or managed care experience (desirable), excellent communication and analytical skills. Must possess a valid driving record and reliable transportation.
