Job Description:
Assessing clients\' needs, plan and link appropriate services, advocate for and monitor client needs to ensure the provision of comprehensive services meet identified client needs. Specific (Essential) Responsibilities: Perform assessment of client\'s physical, mental health and social needs, life skills competency, employability, educational needs, quality of life issues, strengths and needs, and other necessary areas. Based on the Service Needs Intensity Assessment tool, meet with assigned clients regularly and provide appropriate services. In Collaboration with the Housing Resource Coordinator, communicate and establish working relationships with local landlords to negotiate lease terms and advocate on behalf of clients. Work closely with mental health staff to screen clients for referral to Mental Health or Substance Abuse services. Conduct Basic Life Skills workshops at the direction of the Assistant Director. Coordinate delivery of all services to clients. Assure all relevant information pertaining to clients is disseminated to all appropriate staff members in a timely manner. Encourage the client and his/her family to focus their efforts on attaining the objectives specified in the service plan. Assist clients in securing appropriate services. Maintain complete client records in accordance with agency and best standard practice.
