Job Description:
The Case Manager will work with clients with complex psychosocial issues including those who are HIV positive and/or homeless, substance using, mentally ill, disabled, or otherwise in need of intervention. Job functions will include intake, assessment, development of written collaborative care plans, care coordination, counseling, advocacy, referrals, follow-up, and documentation in client medical record. The Case Manager will provide services on site at the clinic as well as broker services with and on behalf of the client while in the community. The Case Manager will also work closely with Mental Health and Primary Care clinicians in coordinating care.Work Schedule/SalaryThe work schedule is Monday - Friday, full-time, with some evening hours required. Salary is commensurate with experience. This is a fully benefited position.Education and Skills:o MSW preferred. Minimum of a BA/BS degree in related field which included field work experience.o Knowledge of current local, state and national resources, services, and benefits programs. o Ability to deal with difficult clients and de-escalate those who may have difficulty managing their own behavior due to emotional, mental or physical illness and/or substance abuse; o Ability to work within the complex multi-cultural environment of San Francisco's neighborhoods (Tenderloin, Mission, Civic Center).o Solid understanding & ability to explain HIV disease transmission, testing, prevention, & course of disease.
