Job Description:
Support individual by providing assistance and training in all areas of daily living including, but not limited to, the following: Self-care, grooming, hygiene Care of clothing, laundry Cooking, nutrition, dietary needs, meal preparation, menu development, grocery shopping Household maintenance, cleaning Budgeting, banking, comparative buying, financial documentation Leisure activities, recreation Appropriate social behavior Utilizing various modes of transportation for community integrated activities Awareness and use of community resources 2. Participate in the individual's program planning process by: Attending and contributing to annual and ancillary meetings, as scheduled Assuring individuals' rights consistent with agency policy Implementing program goals Documenting goal progress Completing monthly reports Supporting individuals' independence as defined in their program plans 3. Assists individuals in maintaining good health by: Assuming knowledge of an individual's medical history and current health issues Assuming knowledge of an individual's physician and ancillary health professionals Accompanying an individual to medical appointments or emergency service, as necessary Communicating with supervisory staff about the health needs of an individual 4. Assures individuals' safety and a safe home by: Teaching safety precautions in the home and community (1) Conducting monthly fire drills, which includes checking the fire extinguishers (2) Checking smoke alarms daily (3) Submitting repair request forms for routine maintenance items (4) Notifying supervisory staff of emergency safety issues 5. Address various individual needs by: Communicating with co-workers and the appropriate supervisor Reviewing co-workers' log entries Documenting daily in individual communication logs Relating to families, guardians, advocates, neighbors and other program staff in a professional manner Notifying the on-call supervisor of emergencies via team beeper 6. Supports and assists individuals in maintaining their home by involving the individual in: Shopping for food and other household purchases Preparing meals Managing weekly household expenses Weekly household cleaning
