Job Description:
This position is available in the following counties: Otero, Dona Ana, Sierra, Luna, Grant, Hidalgo and Catron. Primary responsibilities of the Service Coordinator/Care Manager, RN include: Visiting patients in their homes, assisted living facilities and other community-based settings Delivering a holistic approach to coordinated care based on member needs Identifying early risk factors and conducting ongoing assessments Creating care plans and focusing on disease management Functioning as an advocate for members and decreasing unnecessary hospitalizations Collaborating with the members Physician to deliver and coordinate necessary services Building relationships with members and their families; assisting them with proper health choices Ensuring cases are documented in a timely manner Participating in team meetings Requirements:The requirements for this role are as follows:An RN with a current and unrestricted license in their state of residence Substantial clinical experience
