Capital BlueCross seeks a PA-licensed RN to coordinate case management services for geriatric members, with an emphasis on promoting quality care and cost effective outcomes. You will use a collaborative process to assess member needs; review service/resource options; develop/implement care plan; monitor/document progress; and participate in development of programs within clinical management to meet the needs of our geriatric population. This position requires 3 years of recent community health based geriatric clinical experience; PC skills (MSWord/Access); ability to develop/design clinical programs targeting geriatric population; a PA driver\'s license; and access to own vehicle for travel when necessary
