Job Description:
Ma and is part of Harbor Health Services, Inc with corporate offices located in Boston. The Dental Case Manager works closely with dental providers, primary care and social service providers at the health center to coordinate dental services for people with HIV/AIDS. Duties include but are not limited to initial intake, coordinating patient care within the primary care, behavioral health and dental departments; performing counseling and testing; and in accordance with program specifications, ensures that billing is processed appropriately. Qualifications: High School Diploma or GED, 1 year experience in a professional setting in a human service or related field, bilingual English/Portuguese preferred, computer literate, must be able to work independently and as part of a team, must be able to represent Harbor Health to community groups and outside agencies.
