Job Description:
This key position will be responsible for ambulatory case management at the TMG medical offices; focusing on the severely ill/injured high risk patients who need monitoring and coordination of care, in conjunction with the primary care physician, specialists, and ancillary services providers. Implement, evaluate, and revise outcome-oriented case management plans for individual patients. Also participates in population based disease management programs. Interacts with hospital and health plan case managers, medical staff, data informatics, quality and utilization departments. Position will be based at our corporate office; conveniently located in Costa Mesa with easy access to the 405 freeway.Qualified candidates: current CA RN license, Bachelor´s Degree in Nursing, current California Drivers license with minimum three years of acute care and ambulatory clinic experience including catastrophic and transplant case management. Strong working knowledge in State and Federal regulations, HMO, PPO and Utilization processes, strong written and verbal communication skills, strong problem-solving skills and flexibility, excellent interpersonal and organizational skills, demonstrated ability to work independently and self-directed, ability to plan, organize, prioritize, evaluate and document and computer proficient with MS Outlook, Word and Excel.
