Job Description:
The incumbent uses interviews to establish facts about the veteran\'s condition, the environment, available support, eligibility, and impact of circumstances on the veteran and recommends or makes referral to appropriate program(s) or directs veteran to appropriate community resources. The incumbent establishes procedures and coordinates the screening and admission process. The incumbent provides counseling to the patient, caregiver(s), family members, and/or significant others assisting them in coping with eh psychosocial stress related to illness and other stressors. The incumbent develops psychosocial treatment plans, actively involving the veteran and their family or significant others, in coordination with interdisciplinary team members, based on the psychosocial assessments, including goals for clinical treatment. The incumbent assists veterans in maintaining appointments at VA medical centers and other community agencies for medical, psychiatric, substance abuse, vocational and other services consistent with veteran\'s goals associated with healthful independent living. The incumbent provides referral to VA medical and mental health inpatient and outpatient treatment and to other VA services including VA benefits, as well as community-based social services and other non-VA entitlement program. The incumbent establishes a continuing relationship with the veteran, evaluating progress towards goals and adjusting the treatment plan as appropriate. The incumbent makes recommendations for care and changes to interdisciplinary treatment plan. The incumbent documents independently in the medical records utilizing paper and/or electronic formats, including psychosocial assessments, diagnostics, treatment, progress notes, follow-up and referral.
