As the Care Manager, you would spend part of your time visiting patients in their homes in your assigned territory, and the rest of the time working from your home office. Primary responsibilities include: Conducting in-home visits for high acuity patients (Medicaid members) Working with physicians and family members Arranging care Developing treatment plans Monitoring care Assisting members in navigating through the care system Participating in conference calls and attending office meetings as needed Requirements: Requirements include: A current, unrestricted RN license in your state of residence A clinical background (preferably in Med/Surg, ICU, Home health or Medicare/Medicaid) Excellent organizational skills In addition, ideal assets include: Fluency in English/Spanish or English/Vietnamese Case management experience.
