Registered Nurse - OIF/OEF Case Manager Major Duties: The Case/Care Manager, is a member of the Case and/or Care Management Program, and reports to the Jesse Brown VA Medical Center (JBVAMC) OIF/OEF Program Manager. Responsibilities include, but are not limited to: acquires and demonstrates clinical expertise and knowledge of health care programs to improve the quality of patient care, the use of resources, and to facilitate compliance with internal and external requirements and standards; acquires and demonstrates strength in the areas of interpersonal relations, and conflict resolution; collaborates with other case/care managers, the Interdisciplinary Treatment Team and other clinical and administrative staff as needed to ensure patient care needs are met; organizes and coordinates patient care across the care continuum and achieve patient outcomes in collaboration with the interdisciplinary team; provides clinical case management of OIF/OEF service members and veterans as well as patients in the Traumatic Brain Injury (TBI)/Polytrauma clinic; collaborates with other VA facilities, Military Treatment Facilities, and various referring facilities, etc. The incumbent will be assigned to the OIF/OEF Program with primary duties in the JB VAMC Community Based Outpatient Clinics (CBOCs) as case management, on call, and coverage duties in the OIF/OEF Program. Other duties include, but are not limited to: case management, skilled nursing and intervention for OIF/OEF outpatients; phone call triage/management for OIF/OEF outpatients and significant others; patient education; treatment planning; complete intake screening, psychosocial assessments, clinical reminders; needs assessment; monitors patient facility admissions and assists with discharge coordination; participates with performance improvement initiatives and collecting relevant data required for Commission on Accreditation of Rehabilitation Facilities (CARF) and Joint Commission collects, tracks, and trends data for designated population; etc. The position may require intermittent unplanned travel to escort seriously ill service members between VAMC and Military Treatment Facilities via medivac flight or other travel modes. U.S. citizenship required. Nurse graduate from a NLNAC/CCNE accredited school of nursing. Current, active, unrestricted license as a registered nurse. BSN required. MSN or a related field preferred. 5 years of successful nursing practice required. Successful completion of the Veterans Health Initiatives Traumatic Brain Injury Course is required 90 days upon hire. Training provided.
