Performs BHA case management within the scope of licensure for members with behavioral health care needs. Conducts assessments to identify individual needs. Identifies specific case management plan to address objectives and goals as identified during assessment. Implements care plan by facilitating authorizations/referrals as appropriate within benefits structure or through extra-contractual arrangements. Coordinates internal and external resources available to meet identified needs. Monitors and evaluates effectiveness of case management plan. Modifies case management plan as necessary. Assists in problem solving providers, claims or service issues. Qualifications Minimum of 3 years of clinical experience in Social Work, Counseling, Psychology or Nursing with current, unrestricted California license as a Clinical Social Worker, Marriage and Family Therapist, Clinical Psychologist, or RN. Psychiatric RN preferred. Experience in case management with patients with a broad range of complex psychiatric disorders is required. Ability to comprehend medical policy and criteria to clearly articulate health information is required. Excellent communication and interpersonal skills are essential. Self motivation and the ability to assume a lead role in ensuring that all objectives are met is desired.
