A center serving as a major healthcare resource seeks a Case Manager with 5+ years of clinical experience. The candidate will be responsible for facilitating the patient’s hospitalization from pre-admission through discharge. The candidate coordinates with physician’s, nurses, social workers and other health team members to expedite medically appropriate cost effective care. Applies clinical expertise and medical appropriateness criteria to resource utilization and discharge planning. Will advise the health care team and provide leadership as needed. The candidate should have current NYS RN license/registration, PRI Certification. BSN preferred. Supervisory skills in utilization review or discharge planning is essential.
