Job Description:

A not-for-profit health care provider seeks a Case Manager with 3-5 years of clinical experience. The candidate will be responsible for serving as an educator and resource to the medical and hospital staff regarding hospital resource utilization issues. Collaborating with physicians daily regarding patient care course. Facilitating communications between and negotiating with the patient/family, physician, nurses, dietary, rehab and other disciplines to provide care for the patient. The candidate should have current licensure as a registered nurse granted by the state of Wisconsin. Case management or utilization management experience preferred. Position requires working knowledge of the components of quality and acute patient care needs.
