Job Description:

A not-for-profit hospital system seeks a Case Manager. The candidate will be responsible for comprehensively planning for case management of a targeted patient population. Holds joint accountability with social worker for facilitating psychosocial and continuing care. Works with the physicians and multidisciplinary care team to facilitate efficient, quality care and achievement of desired treatment outcomes. Communicates clinical information to payers to ensure reimbursement. Collaborates with decision support and performance improvement to collect and analyze patient care and fiscal data related to target case types. The preferred candidate should have B.S.N; diploma or Associate's degree in nursing (2-3 years' recent case management experience). For nursing unit, rehabilitation and psychiatry case management, B.S.N. or M.S.W. required. 3-5 years' clinical experience is required. 2-5 years' case management experience is necessary. RN license in the State of Texas is essential.
