Job Description:
The Clinical Case Manager is also responsible for the clinical services provided to children and their families through individual, dyad, group, and family therapy. The Clinical Case Manager will work in two group home programs in East Freetown; in one they will provide primarily clinical services and in the other primarily case management services to the children on their caseload. Responsibilities Include: * Maintain daily contact with outside collaterals and families * Relay all significant information to Cranwood\'s treatment team * Attend conferences, family group conferences, planning meetings, or other relevant clinical meetings as assigned by the Program Director and/or Assistant Director * Write comprehensive bio- psycho-social assessments, disruptive analysis, progress notes, discharge summaries and other treatment reviews in a timely manner. * Create genograms and eco-maps for clients * Conduct intakes and meet with families upon intake of child * Conduct a CANS assessment for children in placement * Provide individual therapy to children as needed * Provide family treatment through direct sessions on a regularly scheduled basis, on site or in the homes as determined by the treatment team * Lead therapy groups * Provide clinical consultation to other treatment team members * Provide clinical or administrative on-call services.
